Transcend Virtual Care’s Remote Patient Monitoring (RPM) provides cost-effective monitoring to anyone suffering from one or more chronic conditions. Transcend’s RPM service works with physicians, and is largely paid for by Medicare or other Advantage insurance plans. It supports a longer and more vibrant life by providing predictable health outcomes. Transcend’s RPM is flexible and can be tailored to meet the needs of care facilities, clinics, ACOs and their patients. Through our services, such as our timeline graphs of a patient’s chronic condition readings, physicians can quickly see the outcome of a medication change and feel confident their patient is on the right path. Transcend Virtual Care includes well-thought-out and flexible offerings for physicians to offer their patients. They include:

Remote Patient Monitoring (RPM): This service monitors a chronic condition including hypertension, Diabetes, Congestive Heart Failure, COPD, Fever, Fever from UTIs and much more; reimbursement for a 30-day period must include 16 days with a least 1 reading
Chronic Care Management (CCM): This service monitors 2 chronic conditions
Advanced Primary Care Management (APCM): This new service removes much of the complexity of time-based sessions with patients
Transitional Care Management (TCM): This service helps patients as they transition from hospital care to an interim facility or their normal living situation
Remote Therapeutic Management (RTM): This service is less about physiologic monitoring and more about interaction with providers. It includes Medication Adherence which provides adherence sensors that can monitor patients’ medications. These cellular-enabled sensors can track whether patients have taken their medications on time and also assess whether they have taken the correct ones
Primary Care Management (PCM): This service is designed to remotely monitor a single condition. A physician with a patient with 3 chronic conditions could monitor 2 conditions with CCM and the 3rd with PCM
Behaviorial Health Integration (BHI): This service combines digital behavioral health assessment with an advanced analytics engine to rapidly identify patient behavioral health issues
Medication Therapy Management (MTM):Medication Therapy Management is a patient focused approach that involves comprehensive evaluation and management of the various parameters in a patient’s medication regimen. This service is offered by pharmacists and other healthcare professionals in order to optimize medication therapy and improve therapeutic outcomes for patients

We Monitor Vitals In The Background And On Demand

Transcend RPM is a powerful monitoring solution that supports continuous patient monitoring, via FDA approved, medical grade, cellular devices, for multiple conditions simultaneously. We work with our Care Provider partners, their licensed nurses and staff to support a longer and more vibrant life. A patient’s physician will set up a Care Plan for monitoring the patient’s vitals. If at any time, a vital sign that is being continuously monitored exceeds the maximum condition set by the patients physician, physician and staff will be alerted on Transcend’s RPM Dashboard. Their staff has the option of contacting the patient by video, phone or text to inform them of the issue and check to make sure they are ok.
